What can go wrong during a change of holder?

1. The wrong domain name was transferred because:
a) The domain holder specifies the wrong domain name on the transfer form or through an equivalent procedure.
b) The registrar submits the wrong domain name to Registry Services.

2. A person other than the domain holder initiates the change of holder. This could be:
a) An employee who is not authorised to do so.
b) Someone impersonating the domain holder.
